Baptist Health Oncology/Hematology

Full Time 40 hours weekly dayshift

Job Summary:  

Under the direction of the Oncology Program Manager, the Oncology Social Worker is responsible for providing psychosocial services in collaboration with patients, caregivers, multidisciplinary team members and professional providers for Baptist Health cancer patients and their families.

Principal Duties and Responsibilities:

The following is a summary of the major functions of this individual's job.  He or she may perform other duties, both major and minor, which are not mentioned below, and specific functions may change from time to time.

1. Provides for systematic patient/family centered psychosocial screening and assessment.

2. Formulates recommendations based on assessed needs.

3. Utilizes evidence-based psychosocial interventions in response to the assessed needs of oncology patients and families.

4. Consistently assist patients and families with navigating the healthcare delivery system for access to services and information.

5. Evaluates and offers instruction in possible coping strategies and problem solving.   Helps patients and families make crucial decisions regarding their care, particularly regarding quality of life. 

6. Coordinates family conferences when needed in an effort to facilitate shared decision making and communication based on patient care needs.

7. Advocates to remove barriers to quality care and address gaps in services available to oncology patients and caregivers.

8. Develops and manages support groups to enhance the quality of care for patients and families.  Organizes resources to address the needs of cancer survivors.

9. Serves as an institutional contact for community and national resources.  Collaborates with community agencies to maintain information on support services that is current and readily available for patients and caregivers.

10. Communicates with team members in writing and orally the elements of the patients and families plan of care including persons and agencies involved.

11. Works with the oncology service line team in developing innovative strategies to serve the educational and support needs of oncology professional staff, patients and families.

12. Collaborates with the interdisciplinary team in communicating assessed needs to care coordinators and discharge planners.

Minimum Education, Training and Experience Required:

Master of Social Work degree required

License in Clinical Social Worker preferred

Current Clinical Social Worker License – KY required

Counseling skills necessary to provide individual, family and group support as needed.

Must be able to communicate effectively orally and through written documentation.

Must function as part of an interdisciplinary team member as well as work independently.
