Ongoing Service Coordinator

Details about this opportunity:
+ Location:Suffolk County, NY
+ Status:Part-Time, Fee for service or Full time Potential. Follows the school Calendar .This position is fully remote.
+ Hours:Available Monday-Friday 8am-4pm (Not necessarily available all day.)
+ Compensation:$13 per unit- $60/Hour
**Please note this is a range. Rates are determined by the service provided, professional experience and license/certifications held. In addition the position posted may be either full-time or fee-for-service (FFS), depending on the current caseload which will be discussed during the interview. This arrangement can vary and is subject to change.**

Responsibilities:
+ Responsible for monitoring the delivery of Early Intervention Services on a child’s IFSP
+ Provide parents with continuing opportunities to share information, priorities and concerns regarding their services and concerns of their child
+ Insure timeliness of paperwork submission and implementation of NYS and County regulations
+ Facilitate the development of a transition plan to preschool
+ Attend and participate in continuing education, ongoing staff in-services and quarterly meetings

And this background:
+ Must be an Established Ongoing Service Coordinator
+ Associates Degree or BA in a Heath or Human Service field
+ Committed to helping children and their families.
+ Ability to multitask efficiently with several cases.
+ Experience in Early Childhood.
+ Obtain BEI Certification for Service Coordination within 90 days of employment.
+ Bilingual is a plus
