Well-Being Navigator - Bilingual - Anaheim CA

The Well-Being Navigator will build trust and rapport with employees and leaders at their dedicated worksite to help facilitate culture change in all areas of well-being. This client-facing role will be onsite full-time with the ability to travel to any of the client's locations at their request.

Responsibilities:

Serve as a wellness benefit concierge, providing referrals and education that support both emotional and physical well-being.

Address the employee’s barriers to care including social determinants of health by helping the employees navigate and schedule all healthcare appointments or connect to community resources to meet their needs.

Provide culturally sensitive care to a diverse group of employees.

In partnership with the wellness team, implement and execute effective wellness solutions to close gaps in care and reduce total plan spend.

Qualifications

Minimum of a (BSW) Bachelors Degree in Social Work, but strongly prefer a Master’s degree in Social Work (MSW)

Bilingual Spanish or Haitian Creole to support client population.

At least 2 or more years in benefits and/or healthcare

Understanding and sensitivity to multi-cultural communities

Excellent communication and interpersonal skills

Strong organizational and time management skills

Ability to document patient interactions in multiple electronic systems.

Work independently with critical thinking skills.

Strong team player and proactive problem solver capable of accomplishing program goals

Ability to maintain member confidentiality.

High degree of patience and commitment to team culture

Ability to work the following hours:

Hours: 8:30 am – 7 pm Monday – Thursday with occasional weekends (Weekly schedule will not exceed 40 hours).

This role is based in the office in Anaheim CA.


If you will be working at home occasionally or permanently, the internet connection must be obtained through a cable broadband or fiber optic internet service provider with speeds of at least 10Mbps download/5Mbps upload.

For this position, we anticipate offering an annual salary of 65,500 - 109,100 USD / yearly, depending on relevant factors, including experience and geographic location.

This role is also anticipated to be eligible to participate in an annual bonus plan.
