OP Nurse Coordinator I (Casual)
**Purpose:** The Outpatient Nurse Coordinator, a Registered Nurse, is an integral member of the care delivery team. They are responsible for overseeing patient care from a holistic perspective, managing various aspects of the patient care package even with minimal direct contact. Utilizing independent judgment and ongoing communication with patients, families, physicians, and community resources, they help patients achieve outpatient health. The role also emphasizes a strong commitment to the community and the nursing profession. This position will work on Tuesday’s and Thursday’s at Wexford, PA’s Pine Center. Additional hours are available if interested. **Responsibilities:** · **Professional Development & Quality Care:** Demonstrates accountability for professional growth to enhance practice and patient care. Actively engages in shared governance and goal setting to improve care quality and the practice environment. · **Team Collaboration:** Serves as a full partner on the care team, responding to team needs and participating in efforts to improve patient care and the professional environment. Adapts to change and provides oversight and supervision as needed. · **Patient-Centered Care:** Formulates patient care goals and plans, involving patients as partners. Triages patient situations, coordinates care with physicians, and provides holistic care. Accountable for outpatient follow-up and serves as the primary nursing resource. · **Relationships & Communication:** Develops productive relationships with colleagues and patients, demonstrating accountability, enthusiasm, and cultural sensitivity. Maintains work-life balance and models safe work hours and healthy lifestyles. Communicates safety hazards and assists in patient care planning and innovations. · **Quality Improvement:** Actively participates in quality improvement efforts, identifies opportunities for improvement, and uses research to support clinical care enhancements. Prepares for regulatory reviews and audits, advocating for patients within the practice. · **Skills & Judgment:** Establishes positive relationships with various stakeholders, works effectively in complex environments, and demonstrates good clinical judgment and critical thinking. Acts as the patient advocate and communicates effectively. · **Critical Thinking & Documentation:** Identifies and addresses clinical, social, safety, psychological, and spiritual issues. Practices solid communication skills, documents patient care comprehensively, and aligns with organizational goals to improve patient safety and satisfaction. · **Education & Development:** Applies adult learning principles in teaching patients, families, and staff. Serves as a primary nursing liaison, supports the development of new staff, and creates an environment of continuous development. · RN Required · BSN preferred · Minimum 3 years of nursing experience required. · The individual must be able to demonstrate the knowledge and skills necessary to provide care and/or interact appropriately to the ages of the patients served by his/her assigned unit as specified below. They must also demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patients status and interpret the appropriate information needed to identify each patients requirements relative to his/her age-specific needs and to provide the care needs as described in the department policy and procedures. **Licensure, Certifications, and Clearances:** Current licensure as a Registered Professional Nurse in practicing state. CPR Certification UPMC approved national certification preferred. CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire · Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR) · Cardiopulmonary Resuscitation (CPR) · Comprehensive Crisis Management (CCMC) · Registered Nurse (RN) · Act 31 Child Abuse Reporting with renewal · Act 33 with renewal · Act 34 with renewal · Act 73 FBI Clearance with renewal*Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state.
