Job Summary:
The Hospital at Home Operations Coordinator is remotely patient-facing and supports all the administrative and logistical functions of the Hospital at Home program. The operations coordinator will work closely with the interdisciplinary team to schedule all care team visits, coordinate supply and equipment delivery to the patient’s home, and arrange patient transportation. The operations coordinator will support all patient logistic operations as required while maintaining high quality and safety standards.
Position Responsibilities:
Supports Hospital at Home leadership and nurse staffing with organizing patient assignments and scheduling of all virtual and in-person care team visits, and supports the Hospital at Home Clinical Coordinator with virtual rounding and in-bound call response.
Arranging, tracking, and maintain inventory of all required patient care resources deployed to a patients home and communicating the status of those resources to various internal and external stakeholders.
Works with the patient and transport services to coordinate any required patient transportation needs. Will also track and escalate delays with patient transportation turnaround times as needed.
Adheres to policies and procedures of the organization, including HIPAA and Patient confidentially regulations.
All other duties as assigned.
Position Qualifications Required:
Required Experience:
Previous administrative support experience in a healthcare setting or direct patient care experience required. Experience working in medical supply chain and logistics preferred. Strong customer service skillset and detail-oriented.
Required Education:
High School Diploma required. Associates Degree preferred.
