MINIMUM QUALIFICATIONS:
Must have excellent communication and interpersonal skills Must be fluent in the English language: reading, writing, speaking Must be willing and able to work weekends and holidays if needed Must have basic computer skills in Microsoft Office Suite (Excel, Outlook, and Word) and electronic medical records Successful completion of the employment screening process to include, but not limited to, drug test and national background investigation
PREFERRED QUALIFICATIONS:
Bachelor's degree from an accredited college or university (Direct experience may substitute for the degree requirement on a year for year basis) Experience with Medicaid eligibility in a long-term care facility One year of experience in a health care setting as a social worker
POSITION DESCRIPTION:
The incumbent works under the supervision of the Business Manager and is responsible to provide social services to meet the individual needs of residents.
Complete intake forms on all residents upon admissions Provide knowledge of Medicaid benefits to residents Coordinate and complete assessment appointments Track progress and renewals for new residents and existing residents Complete and submit Medicaid applications and renewal applications Prepare monthly reports for billing Complete documentation gathering, faxing, scanning and copying Perform other duties as assigned.
