Job Summary:

Under the supervision of the Regional Chief of Optometry, examines eyes to determine visual status & performance by means of instrumentation & observation, & prescribes corrective procedures.



Essential Responsibilities:

• Examines patients having a full range of visual problems by taking patient’s case history & reviewing medical records, if available
• Examines patient’s eyes & adjoining structures for evidence of pathology
• Investigates the state of patient’s visual functioning by using a full field of optometric instruments & techniques
• Diagnoses visual deficiencies & prescribes corrective lenses
• Prescribes & fits contact lenses
• Determines visual requirements of the partially sighted
• Prescribes low-vision aids such as telescopic & microscopic lenses, magnifiers & other optical devices to aid low-vision patients
• Prescribes & fits contact lenses

• Examines patients presenting w/ ocular emergencies
• Prescribes medications when appropriate
• Refers patients w/ indications of possible ocular pathology to an Ophthalmologist as needed
• Refers patients to other appropriate KP providers as needed for further evaluation & required treatment
• Participates in departmental & educational meeting including peer review activities within the department & medical facility
• In addition to defined technical requirements, accountable for consistently demonstrating service behaviors & principles defined by the KP Service Quality Credo, the KP Mission as well as specific departmental/organizational initiatives
• Also accountable for consistently demonstrating the knowledge, skills, abilities, & behaviors necessary to provide superior & culturally sensitive service to each other, to our members, & to purchasers, contracted providers & vendorsSupports compliance & Kaiser Permanent’s Code of Conduct by adhering to federal & state laws & regulations, accreditation & license requirements, by policies & procedures

• Graduation from an accredited college of optometry w/ a Doctor of Optometry degree,

License, Certification, Registration

Optometrist License (Colorado)


Optometric TPA Certification (Colorado)


Optometric DPA Certification (Colorado)


National Provider Identifier required at hire
