Job Description:
Community-Based Education and Case Management
- Maintains an active caseload providing interventions as needed and within area of expertise and limits of credentials; assures procurement of additional services as needed.
- Acquires and provides to the team detailed information regarding an assigned client to establish the foundation for the Treatment Plan.
- Works with the client and their family using professional judgment and discretion to implement the team-determined Treatment Plan.
- Assists in the development, implementation and revision of individual treatment plans; assures that services provided are specified in the Treatment Plan and monitors progress toward treatment goals.
- Provides tailored oral hygiene and nutrition education to children and families, making linkages for interventions as appropriate.
- Assist families in enrolling their child in Medicaid and completing forms, as needed.
Community Based Clinical Navigation
- Consults and cooperates with community systems to facilitate linkage, referral, advocacy, and follow up with the focus on attaining treatment goals.
- Develop engaging strategies with school personnel to inform students and parents about school-based dental outreach activities.
- Establish and maintain close relationships with safety net organizations and have good working knowledge of key contacts, eligibility criteria, referral process, and follow-up outcomes of these organizations.
- Organizes transportation for patients transferred to or seeking services at NCEF Pediatric Dental Center or Healthcare Network facilities.
Administration
- Attends meetings as appropriate and meet regularly with supervisor to exchange pertinent information and receive supervision.
- Prepares and maintains client and program records in accordance with applicable standards and regulations, grant requirements, etc.
- Verifies patient Medicaid eligibility.
- Participates in continuing education activities, remaining knowledgeable in area(s) of expertise.
- Completion of training and ongoing certification as required.
Expected Salary:
$52,200; commensurate with education/experience
Minimum Requirements:
Master's degree in an appropriate area; or a bachelor's degree in an appropriate area and two years of relevant experience.
Preferred Qualifications:
- CPR certification
- Ability to drive to various locations within Collier County
- Good communication skills to foster relationships with partnerships of outreach program
- Experience working with pediatric populations
- Exhibits cultural sensitivity with diverse racial, ethnic, and socioeconomic populations
- Past experience working in the Medicaid system
- Past experience working in a dental outreach program
- Familiarity with community resources, social services, and healthcare in Collier County
- Ability to properly demonstrate and communicate health education activities
- Fluent in Spanish or Haitian Creole
