Community Based Clinical Navigation - Consults and cooperates with community systems to facilitate linkage, referral, advocacy, and follow up with the focus on attaining treatment goals. - Develop engaging strategies with school personnel to inform students and parents about school-based dental outreach activities. - Establish and maintain close relationships with safety net organizations and have good working knowledge of key contacts, eligibility criteria, referral process, and follow-up outcomes of these organizations. - Organizes transportation for patients transferred to or seeking services at NCEF Pediatric Dental Center or Healthcare Network facilities. Administration - Attends meetings as appropriate and meet regularly with supervisor to exchange pertinent information and receive supervision. - Prepares and maintains client and program records in accordance with applicable standards and regulations, grant requirements, etc. - Verifies patient Medicaid eligibility. - Participates in continuing education activities, remaining knowledgeable in area(s) of expertise. - Completion of training and ongoing certification as required. Expected Salary: $52,200; commensurate with education/experience Minimum Requirements: Master's degree in an appropriate area; or a bachelor's degree in an appropriate area and two years of relevant experience.
