OUD Behavioral Health Care Manager II, Pike Place Clinic

** This is a grant funded position through 12/31/2025 **

Purpose

The Opioid Use Disorder (OUD) Behavioral Health Care Manager II will join a dedicated multidisciplinary team to provide comprehensive support to patient populations with the goal of lessening behavioral health inequities. The OUD-focused Behavioral Health Care Manager II plays a critical role in the coordination of needed services, including effective communication with patients from diverse backgrounds, using a patient-centered and harm reduction approach. The Behavioral Health Care Manager II works with other members of our clinical and medical team to increase access and engagement with needed OUD treatment. They assist in connecting patients with needed internal and/or external resources, provide appropriate symptom screeners and follow-up screenings as appropriate, and support patients in overcoming barriers to accessing treatment. The Behavioral Health Care Manager II collaborates with other members of the care team, supporting patient care through appropriate documentation within the EHR, providing information and gathering data as instructed by providers and clinicians, chart review, and participating in team meetings. With support from clinical supervision and using evidence-based interventions and tools, the Behavioral Health Care Manager II will provide some direct patient care for behavioral health patients in the target population, including supporting those who are referred to external resources. The BHCM II will support BHCMs as appropriate by providing clinical insight and will support team-based care, including through, but not limited to, taking part in clinic huddles, informing clinicians and medical providers when treatment-to-target goals are not being met, following up with patients as needed, and documenting appropriately to support patients and quality improvement goals.

In this position you will:

Provide targeted support for a specified patient population (OUD) Provide patients with information about and connection to resources associated with their care Provide behavioral health interventions under appropriate supervision, including but not limited to motivational interviewing, harm reduction support, CBT, and reflective listening Support appropriate treatment through follow up symptom screeners and coordination of the care team. BHCM II may track scores on appropriate symptom screeners and use other metrics as directed and appropriate to assist clinical team in providing high-quality treatment Consult with medical providers and behavioral health clinicians on treatment plans, barriers to care, etc. Ensure proper documentation of consultations and other forms of team-based care in the EHR Provide needed coordination and administrative support of care plan for the clinical team Assist patients in overcoming barriers to care by offering appropriate support, knowledge, and follow-up and through collaboration with other team members, including providers, social workers, patient navigators, and clinicians Provide follow-up and support for patients as needed, including completing assigned screenings, reminder calls, patient follow up support and assessment as directed by the care team, and connection to resources as appropriate Coordinate outreach, engagement, and care management for all patients assigned to them Coordinate and document communication and treatment planning for the care team as directed by supervisors, providers, and clinicians Assist in monitoring program outcomes, including quality and performance outcomes where relevant Facilitate provider review and approval of assessment and care plans Create and maintain timely records and documentation of patient contact activity and client data in the EHR in accordance with role and Neighborcare standards Participate in care review and consultation meetings, team chart reviews, trainings, and other efforts aimed at increasing engagement in treatment and improving patient access and care as directed by supervisor In partnership with patients, partner in supporting patient goals and provide appropriate advocacy

Required qualifications:

Clinical master's degree OR 2+ years of relevant experience A clinical associates license (LMHC, LICSW) or a licensed therapist with Washington State Dept. of Health

Professional training and/or certification relevant to dept and population (ex. HCAs Certified Peer Counselor: substance use disorder professional, harm reduction training, etc)

Preferred qualifications:

Master’s degree in social work 1 year experience in community support, community mental health and/or outreach 1 year providing clinical support for a systemically underserved population 1 year lived or community experience with the targeted population Bilingual/bicultural

Compensation  

The wage range for this position is a minimum $34.17/hour up to $47.41/hour maximum 

Compensation will be determined based on years of relevant experience
