Pay Rate: $50-$55 (dependent on experience) SPA 3: Pomona Amergis Healthcare Staffing is seeking a Disease Management Care Coordinator (DMCC) to assess member’s current health status and understanding of their chronic condition. The DMCC provides education to assist with self-management, provides information on network access, and identifies and implements interventions and resources that address and close gaps in care.
