**Position Highlights**
We are seeking a dedicated and highly skilled Advanced Practice Provider (APP) to join our multidisciplinary team in providing exceptional care for patients in our cardiology and vascular outpatient short stay unit. The APP will play a critical role in managing patients across invasive/interventional cardiology, electrophysiology (EP), vascular, and heart failure specialties. This position is designed to enhance continuity of care between outpatient and inpatient services while ensuring efficient and patient-centered outcomes.
**Responsibilities**
**Clinical Care**
+ Conduct thorough histories and physicals (H&Ps) for patients in the outpatient short stay unit.
+ Update and document progress notes to reflect changes in patient status.
+ Evaluate and manage complications or urgent/emergent issues that arise in the outpatient setting.
+ Discharge patients from the outpatient short stay unit, ensuring comprehensive instructions and follow-up plans.
+ Prescribe medications and provide patient education on medication use, potential side effects, and interactions.
+ Educate patients and their families about cardiac conditions, pre-operative education for all specialties, and post-interventional/operation care, empowering them to actively participate in their health care.
+ Document patient encounters, assessments, treatments, and interventions accurately and timely in the electronic health record (EHR), ensuring compliance with regulatory and organizational guidelines.
**Coordination and Communication:**
+ Collaborate with North Central Heart inpatient teams, including electrophysiology (EP), structural heart/interventional, to facilitate seamless transitions of care.
+ Coordinate follow-up testing, procedures, and clinic visits as needed, ensuring timely and appropriate care.
+ Serve as a liaison between the short stay unit, inpatient services, North Central Heart Clinic and referring providers.
**Specialty Care Management:**
+ Provide focused care for patients across multiple cardiology and vascular disciplines, including heart failure, interventional cardiology, electrophysiology, and vascular conditions.
+ Assist in triaging and managing patients requiring urgent evaluations or interventions.
**Quality and Compliance:**
+ Ensure adherence to clinical protocols and evidence-based guidelines.
+ Participate in quality improvement initiatives aimed at enhancing patient outcomes in the short-stay unit.
**Qualifications**
- Current licensure as a Nurse Practitioner (NP) or Physician Assistant (PA).
- Certification in acute care or a related specialty is preferred.
- Minimum of 2 years of experience in cardiology, vascular, or a related field (preferred).
- Strong clinical assessment and decision-making skills.
- Excellent communication and coordination abilities to work effectively within a multidisciplinary team.
- Experience managing patients in both outpatient and inpatient settings is an advantage.
- Proficiency in interpreting cardiac diagnostic tests, such as electrocardiograms, echocardiograms, and stress tests.
- Strong clinical assessment and diagnostic skills, with the ability to make informed decisions and develop appropriate treatment plans.
- Excellent communication and interpersonal skills, enabling effective collaboration with patients, families, and interdisciplinary teams.
- The ability to adapt to a fast-paced and dynamic work environment, demonstrating flexibility and a positive attitude.
- Competence in using electronic health records (EHR) and other clinical software applications.
- Commitment to providing patient-centered care with a focus on empathy, compassion, and shared decision-making.
