Coordinates the assessment, treatment planning, and managing the disposition of patients including appropriate transfers depending on individual patient's needs. Establishes work priorities, and assists staff in resolving operational issues/problems. Enforces adherence to department, affiliate, operating unit, and/or system standards, policies and procedures, TJC standards, and applicable laws and regulations. Collaborates with others to coordinate services, streamline and standardize work flows and procedures, and/or drive positive outcomes. Makes daily rounds with patients to ensure appropriate patient care is provided, and to conduct service recovery as necessary. Serves as a clinical resource to team, providing direction for issue/problem resolution and responding timely to escalated issues. Develops and implements standard work, processes and workflows to ensure effective and efficient operations while soliciting feedback from staff. Promotes efficient and effective communications between departments to improve and standardize work flows. Collaborates with others to coordinate services, streamline work flow, standardize procedures, and/or drive positive outcomes. Identifies areas of non-compliance or vulnerability, developing, recommending and implementing corrective action plans to address or minimize risk. Keeps leadership informed of operations, and escalates complex issues requiring higher level direction. As necessary, responds to emergencies and/or calls from staff after regular operating hours. Supervises assigned staff, makes or provides input into hiring and termination decisions, develops work schedules, and reviews and approves timekeeping records. Evaluates staff performance and recommends associated merit increase. Provides constructive feedback, coaching and counseling. Implements disciplinary actions and/or performance improvement plans to achieve desired performance. Works with leadership when major disciplinary action is necessary and, if appropriate, in consultation with Human Resources. Responds timely to alleged violations of policies, procedures, regulations and standards of conduct by evaluating or recommending the initiation of investigative procedures. Ensures staff maintains current and appropriate professional credentials. Ensures maintenance of patient’s rights and safety needs. COMPLIANCE: • Maintains strictest confidence of all patients protected health information (PHI) and protects all PHI from accidental, intentional, or inappropriate disclosure. • Performs care in compliance with department standards, policies, and procedures, as well as local, state and federal regulatory agencies to deliver the highest level of patient privacy and care. • Retains certification and completes mandated continuing education requirements to meet facility and regulatory agencies accreditation and licensure requirements. **EDUCATION:** Master's: social work, counseling, or related field. **CERTIFICATION & LICENSURE:** LCSW-Licensed Clinical Social Worker OR LMFT-Licensed Marriage & Family Therapist OR LPCC **TYPICAL EXPERIENCE:** 5 years of recent relevant experience. **SKILLS AND KNOWLEDGE:** Knowledge of administrative and clinical areas with special emphasis on hospital and/or other healthcare settings and standards of patient care. Excellent written and oral communications skills, along with comprehensive knowledge and understanding of regulatory requirements, including The Joint Commission (TJC), California Department of Public Health (CDPH) and Title 22 specifications. Knowledge of medical terminology to include diagnostic and operative procedures. knowledge of State, Federal, and Profession Industry Standards pertaining to healthcare practice. Conceptual, analytical, and problem solving skills in a complex environment.
