Outpatient Nurse Navigator (LPN or RN)

MAIN FUNCTION:  The Outpatient Nurse Navigator will be responsible for performing activities that support the clinical, business, and program related work of the Accountable Care Organization (ACO) clinical staff in the Outpatient Care Coordination department.  The Outpatient Nurse Navigator works with the Accountable Care Program provider offices, inpatient teams and other clinical and non-clinical staff across multiple hospital departments.  The Outpatient Nurse Navigator primarily responsible for helping patients, practitioners, and hospital employees navigate the health care system and support care management activities and readmission prevention initiatives.  He/she performs a variety of complex administrative duties, works with others to identify the appropriate available resources for beneficiaries, and functions as a key contact for ACO activities. 

EDUCATION:  LPN or RN License in CT required

EXPERIENCE:  Minimum 3 years of nursing experience in an acute care, post-acute, or home health capacity.  Demonstrates knowledge of regulatory guidelines.  Proficient in identifying measurable functional rehab goals.

 

OTHER QUALIFICATIONS:  Must be able to work collaboratively as a team member and have excellent interpersonal skills.  Must be able to incorporate self-care and shared decision making in all aspects of beneficiaries’ care while exercising sound judgment in intervening with physicians and other clinicians when appropriate.  Must be proficient using Microsoft Excel and able to learn quickly learn to use new computer software programs.

 

PHYSICAL DEMANDS/WORKING CONDITIONS: Ability to travel frequently to hospitals, skilled facilities, patient homes, and to other sites where patients receive care as indicated by patient need.
