Outpatient Orthopedic Physical Therapist GENERAL DESCRIPTIONConducts appropriate physical therapy evaluations and provides follow-up care as directed by the physician when required. Completes necessary reports and paperwork during regulated time frames. Responsible to Director of Rehabilitation Therapies. Serves patients of ages infant through geriatric.CHARACTERISTIC DUTIES1.Evaluate patients. (See standards for P.T. evaluations)2.Determine appropriate treatment recommendations.3.Identify functional deficits/limitations.4.Establish goals and plan of care to improve patient's function.5.Provide appropriate therapy.6.Complete necessary reports and paperwork.7.Discharge patient with appropriate recommendations and preventative exercises.8.Communicate to doctor or any other professional or family member.9.Educate patient and family regarding their therapy program.10.Provide follow-up to physician and referral source.11.Refer patient to other health professionals as indicated.12.Other duties as assigned.PERFORMANCE STANDARDSCompletes and submits patient charges within defined time frames established by Broadlawns Medical Center (BMC). Documentation is complete and legible at the time of service. Completes annual education and training as defined by BMC (HealthStream, BLS, etc.) Consistently follows service excellence standards as defined by BMC and Rehabilitation Therapies. Completes annual competencies for skills and age appropriate care.MINIMUM QUALIFICATIONS1.Licensed as a Physical Therapist in the State of Iowa.2.Ability to communicate effectively and work cooperatively with other disciplines and departments.3.Ability to develop, implement and evaluate physical therapy treatment.4.Ability to communicate effectively with patients and family ages infant to geriatric.Work Shift
