Outpatient Social Work Care Manager II
About the Job:
The Social Work Case Manager II will support the medical providers, nursing staff, and Neurology clinic patient population in activities related to: social drivers of health, biopsychosocial barriers impacting medical treatment, interoffice consultations with medical providers, patients, and their families on care planning, provide counseling and therapy services with patients as needed
Position Summary:
Provides clinical therapeutic services; to include therapeutic modalities, brief therapy, and case management services to patients and families. Provides general social work services, as necessary, linking patients and families to appropriate referrals and/or assisting them in meeting their needs.
Responsibilities:
Have knowledge of common mental health and substance use disorders Screen and assess patients for social determinants and connect, link, and refer patients to appropriate resources; Collaborate with primary care providers and other Kootenai Health providers, as needed to collaborate and coordinate patient care Complete initial and ongoing diagnostic/comprehensive assessment with each patient Conduct strengths based, patient-centered treatment plans, providing evidenced based interventions and counseling services Support the mental and physical health care of patients on an assigned caseload. Closely coordinate care with patient’s primary care doctor, or others who are involved in the patient’s care, as indicated Systematically track treatment response and monitor patient outcomes Understand insurance payer requirements and processes for billing Meet minimum direct service requirements defined by your manager Complete clinical documentation within required timelines Effective communication and interpersonal skills required Participate in and use clinical supervision and consultation as indicated or required Familiar with standard concepts, practices, and procedures within a particular field Regular and predictable attendance is an essential job function Performs other related duties as assigned Competent to meet age specific needs of the unit assigned
Requirements and Minimum Qualifications:
Master’s degree in Social Work or counseling required Current licensure in Idaho as a LCSW/LCPC, or current licensure in another state as LCSW/LCPC or equivalent AND license in the State of Idaho to be obtained within six months Certification in Case Management preferred Minimum 2 years’ experience providing service in a mental health setting required; hospital or outpatient experience preferred Compliance with accepted professional standards and practices
