We are looking for a dynamic Social Worker to support our Geriatric and Palliative Care Clinic team!

Saint Alphonsus Geriatric and Palliative care Provides education and therapeutic interventions for patients, and families as appropriate to achieve an optimal level of care. Components of the social worker role include relief of stress, crisis intervention, assistance with financial problems, advocacy with community agencies, information and referral, assistance with planning, emotional support, appropriate counseling, and education about family and client's needs to other health care staff and service providers. The goal of social work intervention is the improvement and maintenance of social, emotional, environmental, financial, functional, physical and mental health status of client and family.  Provides care under the general direction of management and with clinical consultation.

 

GENERAL SUMMARY AND PURPOSE:

MINIMUM QUALIFICATIONS:

Bachelor’s degree in Social Work, Psychology, Sociology or other field related to social work.

Licensed by the state of Idaho as a Social Worker

Two years of experience in hospital or medical social work.

POSITION DETAILS: This position will work Monday - Friday 8:00 - 5:00 PM at the Geriatric and Palliative Care Clinic 5966 W Curtisian Ave, Boise, ID 83704.

ESSENTIAL FUNCTIONS:

Knows, understands, incorporates, and demonstrates the Organizational Mission, Core Values, and Vision in behaviors, practices, policies and decisions.   

Demonstrates knowledge and skills to competently care for all assigned age groups (Neonate, Child, Adolescent, Adult, and Geriatric as applicable).

Assesses the client's social and emotional factors related to the illness, need for care, response to treatment and adjustment to care.

Initiates and maintains a liaison relationship as appropriate with all health care providers to assist in effectively coordinating multi-disciplinary care.  

Records problems, interventions, and responses of clients and caregivers according to department procedures and in a timely manner. 

Accurately and promptly identifies counseling needs of clients and caregivers, and provides therapeutic intervention through counseling and education to maximize strengths and coping capacity. 

Manages the patient's financial resources to maximize long-term outcomes.
