Downtown Streets Team (DST) is a nonprofit organization committed to fighting homelessness through the power of work and dignity of community. DST increases social capital and economic mobility by helping our unhoused neighbors chart a way out of homelessness through community building, volunteering opportunities, employment and human-centered case management. Since 2005, DST has expanded across 16 cities in Northern and Central California, serving over 1,500 people annually. SUMMARY: The Outreach Case Manager will connect with individuals experiencing homelessness in encampments, RVs, on the streets and in open space; assessing them for Coordinated Entry and helping them reduce barriers to exiting homelessness through strong organizational, problem-solving, and analytical skills, while managing priorities and workflow. The Outreach Case Manager will need to be able to communicate fluently in English, although bilingual in English and Spanish is preferred. WHAT YOU'LL DO: Educate and roll out best practices for homeless outreach, street-based case management and de-escalation. Coordinate and distribute outreach supplies to individuals in encampments, RVs/vehicles on the streets or in open spaces, within contracted areas. Conduct outreach in San Rafael, Corte Madera, Larkspur, Mill Valley, Sausalito, and Southern Novato. Conduct Coordinated Entry vulnerability assessments to eligible individuals or families. Accurately and promptly document all interactions (phone, meetings, etc.) via case notes with all individuals encountered during outreach. Enroll individuals connected with during outreach into HMIS. Consistently update Tracker, Marin Clarity HMIS, and Oasis for accurate reporting. Tracking impact and reporting in alignment with County contract. Research services outside of the agency that may aid individuals in their goals of exiting homelessness (i.e. St. Vincent de Paul, Ritter Center, Legal Aid). Attend relevant weekly, biweekly or monthly meetings (i.e. encampment case conference, staff meetings) related to County contract or DST. Provide compassionate support and necessary response for individuals in crisis. Screen affected individuals for Covid-19 and work with partners on testing and quarantining. Utilize any available subsidies and affordable housing units to place participants into housing. Establish productive relationships with each and every participant with no bias or prejudice towards race, religion, age, sex, disability, or sexual preference. Practice outreach safety best practices in the field at all times. Must maintain and adhere to written confidentiality protocols. COMMUNITY SPECIFIC RESPONSIBILITIES: In collaboration with second Outreach Case Manager, will connect with unique individuals or families throughout Novato, San Rafael, Corte Madera, Larkspur, Mill Valley, and Sausalito. Encourage but not mandate participation in services. The team shall engage participants by building trust and rapport and embracing a nonjudgmental approach. Maintain a predictable schedule to build trust with outreach recipients. Provide and complete Coordinated Entry assessments when appropriate. Distribute socks, masks, hand sanitizers and hygiene kits during outreach. Support outreach recipients to acquire documentation necessary to enter housing if/when housing becomes available (e.g. photo ID, birth certificate). Connect outreach recipients to resources for which they may be eligible, including but not limited to veterans’ benefits, medical care, employment assistance, credit repair, emergency shelter or other housing supports, etc. Support recipients to manage their personal belongings while unsheltered, including helping them understand any local encampment restrictions, and supporting them with sorting through and disposing of items that they no longer want. Advocate for individuals living in vehicles with working income to be connected to the Rapid Rehousing (RRH) pathway. Report as required to all city and county partners. WHAT YOU'LL NEED: HS Diploma or equivalent. Ability to perform job duties with minimal supervision. Excellent listening comprehension, written and verbal communication skills. Experience in Microsoft programs. Insurance and access to a reliable vehicle. Fluent in English and Spanish preferred. PHYSICAL REQUIREMENTS: While performing the essential duties of this job, the employee is frequently required to do the following: Sitting, standing, walking, bending, stooping, pushing, pulling and lifting (up to 25 lbs.) Using DST assigned computer, phone, keyboard, mouse and monitor. Observation and problem solving; hearing, seeing, reasoning and decision making. Basic mathematical ability. WORK ENVIRONMENT: Moderate noise associated with an open office work environment. Work environment and physical location may vary, depending on the space available and the needs of the community and the program.
