Adult Outpatient Programs provide the least restrictive level of care where adults may receive clinical interventions aimed at decreasing existing signs and symptoms. Outpatient treatment focuses on recovery, problem resolution, symptom reduction, stabilization and/or prevention of movement to a higher level of care. Treatment varies widely and depends upon the consumer’s presenting symptoms.

Responsibilities

Outreach Specialists are responsible for responding to crisis calls, scheduling and attending face to face visits following calls to CarePointe or the Outreach Liaison, educating families on crisis de-escalation and safety plans, and coordinating care with appropriate AltaPointe programs.

 

Essential Functions

Responds to crisis calls in a timely mannerMakes face to face visits when calls come into CarePointe or the Outreach LiaisonProvides services in the community, consumer’s homes, etc.Demonstrates the ability to recognize the elements of a crisis state and knows how to deescalate or resolve the situationEffectively determines the level of response needed in crisis situationsAssists in contacting family members if neededCommunicates effectively with CarePointe to assist in finding appropriate placement when necessaryEvaluates the consumer for community referrals neededEducates families on de-escalating crisis situations including but not limited to safety plansAssists in securing emergency medical services or law enforcement if neededAdvocates on behalf of the consumer to secure community resourcesEffectively collaborates with the treatment team members and outside sources in the delivery of servicesFollows up with consumers that are in AltaPointe’s Adult Outpatient Programs who may be decompensating or at high riskTravels to consumer’s homes to ensure medication compliance as neededUtilizes appropriate communication methods to ensure appropriate communication with individual consumers’ treatment teamsHandles and triages calls from multiple referral sourcesEffectively demonstrates the application of adult specific competenciesProvides transportation to services on a limited basisSeeks clinical supervision and consultation as neededAccepts and employs suggestions for improvementActively works to enhance case management skillsDocuments in a timely fashion per Alta Pointe policyDocuments using Avatar appropriatelyDocuments appropriate, complete, and comprehensive assessmentsDocuments in detail the resolution of crisis situations and linkage to appropriate follow up sourcesDocuments in a clear and concise manner services provided before the completion of the shiftDocuments legiblyEffectively manages caseload based on consumer needsMeets Alta Pointe productivity standardsTreats consumers with care, dignity and compassionRespects consumer’s privacy and confidentialityIs pleasant and cooperative with othersAssists consumers and visitors as neededPersonal values don’t inhibit ability to relate and care for others judgmentsIs sensitive to the consumer’s needs, expectations and individual differencesIs gentle and calm to consumers and familiesActively participates in Performance Improvement activitiesActively participates in Alta Pointe committees as requiredCompletes assigned tasks in a timely mannerFollows Alta Pointe policies and proceduresAttends to required in-service training and other workshops Qualifications

Master’s degree in a mental health discipline.
