Clinical Responsibilities:

 

Responds to crisis calls in a timely mannerMakes face to face visits when calls come into Access to Care or the Crisis Response Intensive Care CoordinatorProvides services in the community, consumer’s homes, etc.Demonstrates the ability to recognize the elements of a crisis state and knows how to deescalate or resolve the situationEffectively determines the level of response needed in crisis situationsAssists in contacting family members if neededCommunicates effectively with Access to Care to assist in finding appropriate placement when necessaryEvaluates the consumer for community referrals neededEducates families on de-escalating crisis situations, including but not limited to, safety plansAssists in securing emergency medical services or law enforcement if neededAdvocates on behalf of the consumer to secure community resourcesEffectively collaborates with the treatment team members and outside sources in the delivery of servicesFollows up with consumers that are in AltaPointe Children’s Outpatient Programs who may be decompensating or at high-riskTravels to consumer’s homes to ensure medication compliance as neededUtilizes appropriate communication methods to ensure appropriate communication with individual consumers’ treatment teamsManages and triages calls from multiple referral sourcesEffectively demonstrates the application of adult specific competenciesProvides transportation to services on a limited basis

 

Supervision and Consultation:

 

Seeks clinical supervision and consultation as neededAccepts and employs suggestions for improvementActively works to enhance case management skills

 

Clinical Record Keeping:

 

Documents in a timely fashion per AltaPointe policyDocuments using Avatar appropriatelyDocuments appropriate, complete, and comprehensive assessmentsDocuments in detail the resolution of crisis situations and linkage to appropriate follow up sourcesDocuments in a clear and concise manner services provided before the completion of the shiftDocuments legibly

 

Case Load Management:

 

Effectively manages caseload based on consumer needsMeets AltaPointe productivity standards if applicable Qualifications

Bachelor’s degree in a mental health discipline, or a nurse, or a certified parent peer specialist with lived experience working with crisis situations.

Must be at least age 23, maintain a valid verifiable driver license and maintain a clear driving record.

 

Knowledge in child/adolescent specialty, outlined below, to be attained within (6) six months of employment:

Knowledge of psychiatric disorders in the child/adolescent populationKnowledge of cultural diversityKnowledge of behavior management, crisis intervention and interventions common to acute psychotic consumers, as well as, to non-violent crisis intervention practiceKnowledge of Psychiatric Rehabilitation principlesKnowledge of available community resourcesKnowledge of educational methods appropriate to children/adolescents with psychiatric disordersKnowledge of substance abuseKnowledge of AltaPointe Health Systems programs and services
