Position Responsibilities:

·       Responsible for producing and working existing enrollment leads generated within Mount Sinai Medical Center health system.

·       Meet with patients, families and others to provide education on the PACE model of care and Eldercare program services and invite those that qualify to enroll in Mount Sinai Eldercare's health plan.

·       Expected to meet or exceed target enrollment goals.

·       Initiate and maintain contact with identified health system departments like care management, physicians, hospitalists, emergency room physicians, behavioral health, rehabilitation, memory care and other departments for direct to patient promotion of the Eldercare (PACE) program.

·       In conjunction with the Dir. Of Business Development develop annual, quarterly and weekly plans that reflect appropriate daily sales calls/staff/patient

·       Maintains accurate account list for all prospective referral sources and tracks all participants through the enrollment process.

·       Attends and participates in Eldercare sales and enrolment meetings and other meetings as directed.

·       Act as a liaison between the patient, health care system and Eldercare as needed to resolve problems, provide information on services, and maintain positive relations.

·       Maintains a working knowledge of applicable Federal, State, and local laws and regulations,  including Mount Sinai and Eldercare policies on compliance, Fraud Waste and Abuse, Code of Conduct, as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical, and professional behavior.

·       Accepts referrals and accurately inputs all information into appropriate tracking methodology(s) and EHR timely.

·       Provides information timely on all new enrollees to IDT and assists as needed to ensure the enrollment process runs smoothly and participant and referral source are satisfied with the process.

·       Participates in Interdisciplinary Team meetings when potential enrollees are assessed for enrollment into the program for purposes of providing and clarifying information obtained during the enrollment process and as needed to be determined by Department and Center Manager.

·       Assists with the financial eligibility process for potential PACE participants by reviewing Medicaid eligibility and assisting prospects with filing the Medicaid application including all of the necessary supporting documentation for approval.

·       Ensures necessary paperwork related to participant enrollment is received by AHCA and DOEA, and Eldercare medical records staff on or before the appointed cut-off date.

·       Collects data and prepares reports in accordance with the AHCA, DOEA, DCF and other agency and CMS requirements.

·       Participates in the quality improvement program.

Qualifications:

·       BLS or CPR Certified. LPN, BSW  preferred.

·       Graduated from an accredited school of nursing (LPN), social work (BSW) or Bachelors degree in health care or marketing related field is preferred. High School or graduation equivalent is required. Education in geriatrics is also preferred

·       Two (2) years of sales or marketing experience is preferred in a health care setting is preferred.  A combination of education and experience may be acceptable.  At least one year working with the frail or elderly
