The Department of Psychiatry & Behavioral Sciences at Montefiore Medical Center is recognized as one of the best in the nation. The Outreach Specialist will assist individuals who qualify for the High-Risk Care Management Program (HRCM). The HRCM program is intended to assist high acuity patients transition from inpatient to outpatient care. The HRCM program is a component of the Behavioral Health Centers of Excellence award.
Outreach specialists will engage with patients prior to discharge across Moses, Wakefield and Weiler Campuses- location may vary based on need. The Outreach Specialist will aid patients in the program complying with scheduled appointments, and will help patients engage and comply with their onsite and offsite psychiatric treatment. When needed, the Outreach Specialist will perform home visits to foster engagement. The Outreach Specialist will aid in initial patient engagement, outreach efforts and accompanying of patients to appointments as needed.
Responsibilities:
• Work closely with patients to aid in engagement in treatment including meeting with patients, calling patients, and accompanying patients to appointments. Outreach patients who have stopped attending treatment at their last know residence
• Actively listen to patients enrolled in HRCM and encourage them to communicate any concerns to appropriate staff on their treatment team. Major issues will be escalated to supervisors
• Document all patient contacts in the electronic medical record, including phone calls, onsite visits and offsite interactions
• Meet with patients in emergency department and inpatient units to assist in engagement and referral to HRCM
• Assist individuals enrolled in treatment with referrals to needed services and connection to resources in the community
• Describe the nature and purpose of HRCM services offered
• Collaborate with a multidiscipinary team in order to build and maintain positive working relationships with staff at all Montefiore Psychiatry Clinics and inpatient units
• Engage with high acuity patients of inpatient units prior to discharge to enroll them into HRCM
• Conduct home visits to foster engagement as needed
• Utilize knowledge of adults across the lifespan to help develop patient-specific goals and provide patient-centered services
Qualifications:
• High School Diploma / GED required; Bachelor’s degree preferred
• Certified Recovery Peer Advocate (CRPA) or CRPA-eligible following completion of internship hours
• Excellent oral, written and interpersonal communication skills. Ability to find creative solutions to challenges
• Familiarity with Microsoft Word and Excel
• Ability to provide outreach to patients enrolled in program, build motivation in patients using empirically supported strategies, including motivational interviewing, and demonstrate ethical behavior and uphold standards
• Must be able to provide emotional support and instrumental assistance to patients enrolled in treatment and assist patients in defining and working toward their recovery goals
Must be able to work at other campuses based on departmental needs.
