Outreach Specialist / Patient Navigator
Research Support Specialist ANNOUNCEMENT OF NON- PROFESSIONAL VACANCY Applications are invited for consideration to the following position Department: Medicine / STAR Program Budget Title: Research Support Specialist Salary Range: (Based Upon Experience)$50,000.00 - $55,000.00 Descriptive Title:Outreach Specialist / Patient Navigator Full-time, Monday through Friday, 9:00am - 5:00pm, 37.5 hours per week. Vacancy 25-3 Description of Duties: This position is grant funded. -Develop health content and conduct mass outreach campaigns to educate and raise awareness about available HIV prevention services, testing, and resources. Engage patients through various channels, including in-person, digital/social media, media, and community events. Maintain tracking log of outreach efforts and outcomes. -Conduct individual outreach with patients wfth positive STI results: patients who would benefit from PrEP; and/or the provider orderingSTI testing with thee goal of educating ancl linking patients to comprehensive prevention services. Provide comprehensive prevention education and facilitate appointments to heath care services (i.e., to addfess STIs, PrEP, PEP, Doxy PEP, Mental Health): Maintain active outreach to all those patients who are referred to care but not successfully linked (ie, do not attend their first HIV, Syphilis, or CPS visit). -Assist with obtaining and tracking syphilis test results and treatment data from the NYC Registry to help inform diagnosis and management of patients with syphilis. -Conduct chart reviews and engage patients, providers, and health department partnerships to obtain patient demographic, health history, risk factors, and care status lnformalion. Assess for financial needs, Insurance coverage, food insecurity, and other service needs and make referrals to address needs. -Assess barriers to linking patients to care: develop, implement and monitor strategies for overcoming barriers to linkage/'retention in care; educate and empower patients. Coordinate logistical support for transportation; Accompany patients to linkage appts as needed. Coordinate care to assist patients in completing applications for Medicaid, ADAP, PrEP-AP, medication assistance programs and other benefits/entitlements: assist patients in obtaining housing, transportation, and also refer patients to CBOs for other needed services. -Document all patient activities in the electronlc medical record and on required data forms and tracking logs in a timely manner; Document consent to call, text, and/or make onsite visit to the patient to maintain contact; Track and report on outreach efforts and participant engagement, Including maintaining accurate records of interactions and assistance provided. Track other relevant program activities and outcomes. -Develop and maintain relationships with internal and external stakeholders, partner organizations, and colleagues to promote comprehensive HIV prevention services. -Participate in STAR Program outreach and education efforts regarding HIV, HCV, PrEP/PEP, and promotion of services; Participate in ongoing quality improvement efforts; Assist with additional duties as assigned. Qualifications: REQUIRED: High school diploma and at least 2 years of experience in outreach, community engagement, health education, patient navigation, case management or a similar field; and at least 1 year of experience in sexual health education, HIV prevention services (such as PrEP/PEP), HIV testing, or related areas. Comfortable discussing sexual health topics with clients both in person and over the phone, delivering positive HIV and STI results, and contacting patients directly, Including making cold calls. Effective written, verbal, organizational and interpersonal skills. Ability to manage multiple priorities, think proactively, and come up with creative solutions to challenges. Must have the ability to work independently and as part of a multidisciplinary team. Proficient in computer use, including typing and using Excel, PowerPolnt, and Word. PREFERRED: Some college education preferred (a 4-year college degree may be substituted for one year of experience). Experience working in multicultural, team-oriented environment preferred. Experience working with people living with HIV, Hepatitis C, syphilis, substance use disorders, or experience with LGBTQ populations preferred. Experience with oulreach and using social media preferred. Experience using electronic health records is strongly preferred. Bilingual In English/Spanish or English/Haitian Creole is an asset. Strong written and verbal communication skills, with the ability to interact effectively with clients and colleagues. Training in Motivational Interviewing, Trauma Informed Care, Hepatitis C Navigation, PrEP Navigation, and HIV Linkage to Medical Care/ARTAS also required, but may be completed upon hire.
