The Outreach Support Specialist provides outreach services to individuals and community agencies that serve Burlington County residents, educating about the different services that Legacy provides. Demonstrates the knowledge of the principles of growth and development appropriate to the consumer population served. Responsible for managing the administrative documents as it relates to the intake process. Completes referral log across all programs within the department. Coordinates with Clinical Director as it relates compliance with CSS documentation. Will provide in-person support services as needed and indicated by program contract commitments and needs. Assists with program development, implementation and evaluation. Attend monthly SRC meetings and community events/ meetings as necessary. Must possess a valid driver’s license and be able to drive an Agency owned vehicle.
Community Support Services offers a variety of supports to help Burlington County adults with a serious mental illness remain living successfully in the community. Services include case management, housing assistance, supportive counseling, advocacy, service linkages, illness management, peer support, life skills, education/employment assistance, transportation and 24-hour on-call support.
