Strong clinical abilities for management of a population age ~55+ with multiple chronic health conditions Patient care settings will include clinic appointments, virtual visits, group visits, taking care to their home and hybrid options. POSITION SUMMARY: The Physician Assistant is responsible for: providing high quality patient care to the patients of OSF. Maintaining the consistency and quality of all services provided to the Practice and its patients. Maintaining full and current patient medical records, including reports of all examinations, procedures, and other services performed by Provider or other support staff. Ensuring interdisciplinary collaboration with digital mission partners, subspecialists and other care entities to ensure a team-based model of care, enhance the patient experience, and close care gaps to drive positive patient outcomes. The Physician Assistant is responsible for providing a truly personalized, patient-centric experience to patients of the practice, OSF OnCall and the OSF Hospital aligned to the patient's preferences, goals and needs. Qualifications REQUIRED QUALIFICATIONS: Education: Master’s degree in physician assistant studies . Experience: Demonstrates the knowledge and skills necessary to provide patient care that is appropriate to the ages of the patients serve. Licensure/ Certification: Physician Assistant Lic Federal Controlled Sub Lic BLS/ACLS Certified Other Skills/ Knowledge: Excellent interpersonal and communication skills. Solid computer skills, including proficiency with Microsoft software. Strong analytical and problem-solving skills, with the ability to be detail oriented.
