*Description* Nuvance Health is seeking a full time *Palliative CareNurse Practitioner *for inpatient and outpatient office based palliative care services at Vassar Brothers Medical Center in Poughkeepsie NY. This is an employed position with Nuvance Health. We offer a comprehensive compensation package, including competitive salary, generous benefits, and allowances for CMEs and Dues. ** *Job Summary:* The primary function of the Palliative Care Nurse Practitioner is to provide medical care for patients in collaboration with the physician. The NP is responsible and accountable for the clinical assessment, management, and evaluation of the adult patients appropriate for palliative care and for carrying a patient case load in collaboration with the palliative care team. The NP is expected to assess/refer patients appropriately for both the physical and psychosocial needs of the patient. The position will report to the Nuvance Health Medical Group Director of Palliative Care. *Job Requirements:* * This position requires a minimum formal education of Master Degree, State of NY RN and NP license, and Federal DEA * At least 1 year experience as an NP, preferably in home based Palliative Care or Hospice Care settings preferred * Excellent communication, clinical and interpersonal skills to facilitate patient care between hospitalists, private physicians, referral and collaboration amongst other healthcare staff. * Able to work well with an interdisciplinary team. * Able to work confidently and independently. * Board Certification in Advanced Certification in Hospice and Palliative Nursing (ACHPN) required within 2 years of hire * Willing to work Monday – Friday 8 am – 5 PM, no weekends or evenings. Willing to take rotating call. Additional duties assigned by Director and team lead.
