What You’ll Do:

Work in collaboration with the Palliative Care Medical Director and other palliative team members to deliver palliative care by providing diagnosis, treatment and follow-up for patients referred to palliative care.Care for patients in their home within a 60 mile radius of our Tigard, OR. officeProvide regular visits to reassess the patient’s needs, concerns, and update the plan of careInitiate communication with the Primary Care physician, other agencies involved in patient carePrescribe medications including controlled substances to the extent delegated and licensed, in accordance with state law.Order and interpret laboratory and other diagnostic tests within the scope of professional practice.Order treatments and durable medical equipment as indicated

Qualifications:

Licensed as a Nurse Practitioner in the state of practiceMinimum of Masters of Science in Nursing from an accredited school of nursing, or an equivalent as allowed under state lawTraining/experience in Palliative Care, Gerontology, Geriatric Medicine and/or Chronic Care preferredPrior Home Health and/or Hospice experience as an RN or NP preferredPractice experience with Part B billing preferredHave current DEA licenseACHPN – Certification in Hospice and Palliative Care PreferredMinimum of two years of varied work experience as a professional nurseMust possess current CPR CertificationDemonstrated skill in nursing practices and clinical judgmentsAbility to make home visits and provide nursing care in the home environmentAbility to cope with family emotional stress and tolerance of individual lifestylesInterpersonal, verbal, and written communication skills necessary to complete job responsibilitiesValid driver’s license and proof of insurance is required
