GENERAL SUMMARY:

The role of the Emergency Services Paramedic is crucial in our organization. They work closely with the House Supervisor Nurse and/or primary nurse to deliver prompt patient care throughout the facility. The Paramedic assists the primary nurse in implementing physician orders for our patient population, always staying within the scope of their licensure. They also ensure a safe and supportive environment for all patients, families, and coworkers.

The role you’ll contribute:

Hospital-Based Paramedic Transfer

Provides primary ALS-level patient care activities on transports approved by the EMS Medical Director

Assesses, plans, implements intervention, and evaluates the appropriate level of patient care according to

Kentucky ALS scope of practice, EMS Protocols, department policies, and procedures, including the following:

• Chronic Ventilator Support BiPAP/CPAP

• Nebulized Medications in handheld or mask nebulizers

• Non-invasive Monitoring (EKG, SpO2, NIBP, EtCO2)

• Transcutaneous Pacer Support

• Non-titratable infusions Blood Products

• Previously established Chest Tubes, drains, tubes, ostomies, or wound vac

• Previously accessed ports, established central venous lines, arterial sheaths, and temporary dialysis catheters.

• Inter-Osseous Placement/Established Access (IO)

• Non-intubated or Mechanically Ventilated post-arrest patients.

• Advanced Airway Providers, ETT, and LMA have manual/temporary ventilatory support, but no RSI exists.

All BLS Therapies as defined by department protocols:

• Monitors, records, and communicates patient conditions and treatments as appropriate

• Educates patients and family members about patient illnesses and clinical interventions

• Inspects and maintains equipment to ensure the safety of transport team members, patients, and the public.

• Monitors and anticipates needs for additional equipment and appropriately secures tools to meet patient care demands during transport

• Operates emergency vehicles safely in various weather and emergent conditions following established safety standards. Effectively communicates delays and updates to dispatch. Immediately report any damage or vehicle issues to the supervisor.

• Responsible for maintaining equipment between transports.

• Maintains the unit in a constant state of readiness

• Adheres to department Standard Operating Procedures (SOPs)

• Ensures patient documentation is accurate, complete, and always complies

• Completes medication, supplies, and equipment inventories each shift, restocking as needed.

• Completes and submits logs as per department protocol

• Performs other duties as assigned

Hospital Based Emergency Service Paramedic

Attach 12 lead EKGs and complete the test before giving the strip (or uploading per protocol) to the physician on duty. Initiate cardiac monitoring and perform venipuncture for testing. Initiate cardiac monitoring, demonstrate the ability to detect dysrhythmias, and notify the assigned nurse or charge nurse of abnormalities. Initiate oxygen therapy as directed by an RN or Physician.

Inserts peripheral IVs, hangs IV fluid without medication, and maintains prescribed IV flow rates. Observe and document the condition of the infusion site. Removes IV Lines as directed by the RN.

Transports unmonitored patients to inpatient departments as needed, transports stable (no blood infusion drips or no oxygen > 50%) monitored patients to PCU/Med-Surg, or accompanies RN to transport patients to specialty units.

Assists physician or RN with procedures and exams (i.e., set up and clean up for sutures, pelvic exams, lumbar punctures, application of orthopedic splints and casting, etc.) Applies slings, sling-n-swathe, finger splints, application of dressings, etc. Follows CDC isolation guidelines, demonstrates appropriate use of PPE, performs hand washing according to policy, and enforces safety regulations.

Is a customer service champion as demonstrated by acknowledging, Introduce, Duration, Explanation, and Thank You (AIDET), keeping patients informed, providing information and updates to patients, apologizing for waits, demonstrating caring, compassion, and concern, establishing a sense of urgency, advocating for pain management, and ensuring that patients can get help at any time. Performs hourly rounding on patients to keep them informed and ensure that they have call bells, pillows, and blankets as needed and that patient and family needs are exceeded.

Performs accurate order entry within Epic.

Provides patient care appropriate to age, mobility, acuity, and culture. Participates in obtaining medical history and allergies. Performs initial focused intake assessment throughout shift, including physical, mental, social, and spiritual. Participates in education and discharge planning using established education materials. Supervised by assigned RN or designee to that care zone.

Assist in advanced cardiac Life Support and PALS under physician direction and perform in the role assigned.

Assist with preparing medication related to ACLS and PALS under the direction of a physician.

Assists with replenishing crash cart supplies and other essential equipment.

Respond to Rapid Response codes as assigned.

Executes verbal orders for medications and treatments limited to protocols, performs documents appropriately, and adheres to Medication Administration policy

Manages phone calls, notifies physician of all back results, orders diets, etc. Performs administrative duties as assigned or directed to ensure the smooth operation of the department or unit.

Performs other duties as assigned.

Medical Management and Treatment protocols are to be supervised by the EMS Medical Director.

Qualifications

LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:

Advance Cardiac Life Support Certification

Basic Life Support CPR Certification

Pediatric Advanced Life Support Certification

Licensed Paramedic with the State of Kentucky
