Holy Cross Home Care & Hospice is a trusted provider of home healthcare services, dedicated to delivering high-quality, patient-centered care. We are seeking a **compassionate and skilled Speech-Language Pathologist (SLP)** to join our team and provide therapy services to patients in the comfort of their homes.
**Job Summary**
As a **Home Care Speech-Language Pathologist** , you will assess, diagnose, and treat patients with communication, cognitive, and swallowing disorders. You will collaborate with a multidisciplinary team to develop individualized treatment plans that enhance patients’ quality of life.
**Key Responsibilities**
+ Evaluate and assess patients with speech, language, voice, fluency, and swallowing disorders.
+ Develop and implement personalized treatment plans based on patient needs and goals.
+ Educate patients and caregivers on therapeutic techniques for continued progress.
+ Collaborate with physicians, nurses, and other healthcare professionals to coordinate care.
+ Maintain accurate and timely documentation of patient progress and treatment.
+ Provide guidance on assistive communication devices when necessary.
+ Ensure compliance with all regulatory and professional standards.
**Qualifications**
✔ **Education:** Master’s degree in Speech-Language Pathology from an accredited program.
✔ **Licensure:** Valid state license as a Speech-Language Pathologist.
✔ **Certification:** Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) preferred.
✔ **Experience:** Previous experience in home health, geriatrics, or a clinical setting is a plus.
✔ **Skills:** Strong communication, interpersonal, and problem-solving skills.
✔ **Other:** Must have a valid driver’s license and reliable transportation for home visits.
