Part Time Case Manager
Part Time Case Manager The Opportunity FCA is excited to announce an opportunity for the position of Part-Time Case Manager for our PACT (Parents and Children Together) program. PACT offers services to young people who are at risk of unprotected sexual activity, early pregnancy as well as pregnant and parenting teens. The PACT program provides teens with opportunities for cultural and educational enrichment activities that can support the development of personal goals. Specifically, the responsibilities and qualifications of the position include: Responsibilities + Provide case management services, and crisis intervention to pregnant and parenting teen parents. Facilitate family life education groups and Prevention workshops. + Provide outreach, casework services, including informal counseling, advocacy, referral and information. + Provide crisis intervention and linkage to appropriate services. + Develop, implement and monitor case management goals. + Complete evidence-based assessment and make appropriate intra and external referrals + Co-facilitate Prevention workshops and webinars + Outreach to Mother/child residential programs, schools, and other community-based organizations in Nassau County. + Conduct home and field visits. + Organize recreational/ socialization activities. Qualifications + Associates degree required, Bachelors preferred + At least one year of experience working with adolescents. + Bi-lingual in Spanish preferred. + Proficient computer skills with MS Office + Excellent verbal and written communication skills + Valid and clean New York State Driver's License + Compensation Information The competitive compensation range is reflected with FCA's framework, culture, and values.
