We are looking for a dedicated and compassionate Home Care Occupational Therapist (OT) to join our home health care team. As an OT, you will provide individualized occupational therapy services to patients in their homes, helping them regain independence and improve their quality of life. You will assess, plan, and implement rehabilitation programs that support patients' daily living activities and overall well-being.

Key Responsibilities:

Patient Assessment & Care:

Conduct initial patient assessments to evaluate functional abilities, range of motion, strength, and mobility.

Develop and implement personalized therapy plans to improve patient independence in daily activities such as dressing, bathing, eating, and other self-care tasks.

Provide adaptive equipment recommendations and educate patients and families on proper use.

Therapeutic Interventions:

Design therapeutic interventions to enhance patients' motor skills, coordination, and functional mobility.

Teach patients energy conservation techniques, fall prevention strategies, and safety measures for the home environment.

Modify home environments as needed to promote safer, more effective care (e.g., grab bars, ramps).

Collaboration & Documentation:

Collaborate with physicians, nurses, physical therapists, and other team members to ensure comprehensive, coordinated care.

Maintain accurate and timely documentation of assessments, care plans, progress notes, and discharge summaries in accordance with company and regulatory standards.

Qualifications:

Bachelor’s or Master’s degree in Occupational Therapy from an accredited program.

Licensure: Current Occupational Therapy (OT) license in the State of Maryland.

Minimum of 1-2 years of occupational therapy experience, preferably in a home care or rehabilitation setting.

Valid driver’s license and reliable transportation for home visits.

Certification in Basic Life Support (BLS) (preferred but not required).
