They provide education with the family and patient. Most of our positions have flexible work schedules that are Monday through Friday which offer a good work-life balance. Our professional clinical staff work in a collaborative environment and have weekly team meetings to discuss the patient’s plan of care.

Responsibilities
The Home Health Medical Social Worker assists the physician, staff, and family in understanding the significant social and emotional factors related to health problems of the patient.
Participate in the development of the patient’s plan of treatmentWork with the family in utilizing appropriate community resourcesParticipate in discharge planning and in-service programsPrepare clinical and progress notes the day of serviceAct as a consultant to other agency personnel Qualifications/Skillsets Master’s Degree in Social WorkMinimum 1 year experience in Social Work in a healthcare settingCurrent CPR CertificationMust have a valid driver’s license and current automobile insuranceMust be a LCSW or CSWA in the StateBasic understanding of age-related differences in caring for and communicating with patients of this typeMust possess the ability to work independently without supervisionEffective problem-solver and use good judgment in difficult situationsMust possess personal sensitivity to the needs and experiences of others and a non-judgmental attitude towards persons of differing standards, values, lifestyles, and ages
