We are currently seeking a Bereavement Counselor in the Los Angeles area who shares our passion for providing dignified, compassionate end-of-life care.
Shift Schedule: Part-Time, 8am - 5pm, 2-3 weekdays

Must have either be a Master Social Worker, Masters of Divinity or related human services field Clinical experience in the area of grief and loss, family therapy and group activities. Must have experience in hospice or palliative care or have palliative care certification One year of experience supervising others to include: setting performance expectations, coaching, motivating, promoting collaboration and teamwork, managing change, providing performance feedback, delegating effectively, resolving conflicts, etc Ability to conduct bereavement support groups Demonstrates ability to plan, organize and make effective decisions Knowledge of regulatory and financial aspects of the hospice program This role requires a valid driver’s license and a good driving record as a condition of employment. Associates must comply with all state and federal driving regulations Associates in roles requiring driving are responsible for maintaining a valid and active driver’s license. Failure to maintain a valid license may result in reassignment, suspension, or termination in accordance with company policies and applicable lawsWhat you'll be doing:
Provide regional oversight and management of the Silverado Hospice and Supportive Care Bereavement Program Conduct bereavement phone calls and develop bereavement plan of care Assign and schedule in-person bereavement visits Document all bereavement activities in our EMR system Ensure cards, educational materials, and bereavement support events are mailed timely Ensure all assigned sites bereavement program meets the Joint Commission Standards and CMS conditions and participation Participate in quality improvement meetings and initiatives Organize Celebration of Life events at least annually Participate in interdisciplinary team meetings#LI-MH1
