**Employment Type:** Part time **Shift:** Day Shift **Description:** **EDDY VISITING NURSE & REHAB ASSOCIATION** Eddy Visiting Nurse & Rehab Association has a rewarding opportunity for a part time Social Worker responsible to manage a caseload of patients requiring social work support through in-home visits. Some patients/families are supported as well through virtual/phone based social work support either in place of in home visits or in combination with in home visits. **Manages a caseload of patients in their homes:** + work with an interdisciplinary care team of nurses, therapists and aides + conduct a comprehensive assessment of patient/family needs + link individuals to the community resources they need to remain in their home or move to a higher level of care if necessary + Once trained, enjoy the independence of starting and ending your day from home with a flexible work schedule + Be on the front-line of the transitions in health care from inpatient settings to the community – home health care is the future! Work schedule is flexible. **Requirements:** + NYS Social Work Licensure, minimum of 1 year of relevant experience in home care, the medical field, work with seniors, etc. required. Eddy VNA provides a comprehensive orientation to home health care. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
