Responsibilities: + Provides professional social work/case management services to residents and families. + Conducts psychosocial assessments for the purpose of admission screening and evaluation of treatment and progress in the applicant's immediate milieu. + Participates in interdisciplinary team staffing to formulate treatment plans. + Provides individual casework counseling and evaluation services to implement specified treatment plans. + Provides intervention services to residents and their families throughout the residents' stay at the facility. + Communicates with relatives, guardians, and other appropriate persons regarding the resident's need for assistance in adjusting, modifying, or improving social functioning and behavior. + Investigates and resolves complaints made by or on behalf of the residents. + Represents residents on behalf of their claims, expenses, and their requests to the facility. + Inputs data with subsequent updates into the MDS Plan of Care for nursing care residents. Minimum Qualifications: + Licensure as a Social Worker in Idaho in accordance with Idaho Code 54-32. MQ Specialties: + Experience in a health care setting or facility social service. Desired Qualifications: + Knowledge of the Minimum Data Set (MDS) process.
