AdventHealth Rocky Mountain region is seeking a PART TIME APP (0.5 FTE) to support the clinical operation of the Transplant Institute at Porter with a primary focus on clinical services in the inpatient and outpatient setting. Delivery of patient care is conducted in collaboration with the transplant surgeons and physicians including, but not limited to, physical exams, diagnosis and treatment, ordering and interpreting tests, counseling on preventative health, writing prescriptions and assisting in surgery.
Under administrative guidance and physician supervision, the APRN/PA will provide comprehensive primary care services, acute care services and extended care services in the area of nursing and/or medicine for individuals and their families.
RESPONSIBILITIES:
Utilizes sensitivity, responsiveness, caring and compassionate attitude with patients, families and visitors by listening and communicating effectively with courtesy, consideration and tact.
Evaluates and treats patients with acute, chronic complaints and health maintenance concerns which include the following:
Assessment of patient health status through physical exam, reviewing patient history, diagnostic tests and procedures and reviewing labs and other critical data.
Diagnosis of acute and chronic conditions affecting all organs systems
Initiating orders for medications, laboratory and other diagnostic testing
Prescribing interventions and performing therapeutic measures to ensure the appropriate individualized plan of care is implemented.
Evaluating patient’s mental status, substance abuse and referring to mental health treatment as clinically indicated
Coordinating care of recipient and/or living donor as needed
Implementing changes in drug treatment therapy as ordered by the physician(s) and/or healthcare providers
Assists in surgery
Attends and participates in transplant team meetings including hospital interdisciplinary rounds, discharge planning and QAPI.
Call responsibilities determined by program-specific needs
Observes, monitors, assesses and documents organ function and general health status as defined by program protocols.
Coordinating information flow among medical, ancillary, nursing, financial and insurance personnel
Assisting with discharge planning in collaboration with the multi-disciplinary healthcare
Participating with the ongoing education of internal and external stakeholders in the care of transplant and advanced disease-state patients, including, but not limited to:
Participating in the development and implementation of protocol and standards of care for the transplant patient in collaboration with the multidisciplinary team.
Overseeing and providing education to acute care staff (ED, OR, ICU, surgery step-down unit) and community care services.
Educating nurses and other multidisciplinary team members about patient care issues to promote positive outcomes for transplant patients.
Pro-actively initiates quality improvement ideas/suggestions in an effort to: 1) improve the patient experience of care (including quality and satisfaction), 2) improve the health of the populations served; and 3) reduce the cost of clinic care.
Assists with the on-boarding, training and professional development of new ARNPs and PAs and mentorship of select individuals within the transplant programs.
Actively participates in regulatory-related data collection and surveys agencies.

EDUCATION AND EXPERIENCE REQUIRED:
A minimum of two years of experience in a large hospital or clinical setting
If Nurse Practitioner - must have successfully completed undergraduate and Master’s level-nursing programs, passed state boards, and achieved ARNP status. (Individuals who completed their training prior to January 1, 2001, shall not be required to hold a Master’s degree.)
If Physician Assistant - must hold a baccalaureate degree, or a minimum of two years of college credits, and have graduated from a physician assistant program approved by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA), the successor program to CAAHEP
LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:
Current BLS and ACLS certification
Current, valid State of Colorado license
