PATH Outreach Case Manager Homeless Services

POSITION PURPOSE:

The PATH (People Assisting The Homeless) Outreach Case Manager is responsible for providing comprehensive support to individuals experiencing homelessness.This role involves outreach, case management, and coordination of services to help clients achieve stability and improve their quality of life.Will work directly in the community to connect with individuals, assess their needs, and provide tailored support and resources.

WHAT YOU WILL DO:

Outreach and Engagement:

Conduct outreach activities in designated areas to identify and engage individuals experiencing homelessness.Build and maintain relationships with clients to foster trust and encourage participation in services.Provide immediate support and resources to individuals in crisis situations.

Case Management:

Develop individualized case plans based on client needs, goals, and preferences.Assist clients in accessing resources such as emergency shelter, housing, healthcare, and social services.Monitor client progress and adjust case plans as needed to ensure effective support and goal achievement.

Resource Coordination:

Collaborate with community organizations, service providers, and government agencies to coordinate services and referrals.

Documentation and Reporting:

Maintain accurate and confidential records of client interactions, case notes, and service outcomes.

Community Engagement:

Participate in community meetings, networks, and coalitions to promote awareness of homelessness issues and advocate for client needs.Collaborate with other service providers to enhance the effectiveness of the homeless service delivery system.

MINIMUM QUALIFICATIONS:

Master’s Degree in Social Services with 1 year of social service experience ORBachelor’s Degree in Social Services with 3 years of social service experience ORAssociate degree in Social Services with 4 years of social service experience ORHigh-school diploma/GED and 8 years of social service experienceAct 33/34/73 clearancesValid Driver LicenseAbility to drive an organization vehiclePre-Employment Drug ScreenStrong interpersonal and communication skills, with the ability to build rapport and engage with individuals from diverse backgrounds.Physical Requirement:  Ability to conduct outreach activities in various environments.

POSITION HIGHIGHTS/BENEFITS

WORK SCHEDULE: This is a Non-exempt (hourly) position.   Monday through Friday 8:00 am until 4:00 pm with flexibility to meet consumers’ needs.
