Job Description:PATH Social Worker is part of a team designed to effectively assist patients in transitioning from hospital to community. The PATH Social Worker will focus durable medical equipment, outpatient therapy, and caregiver training needs to transition between hospital and community-based providers and arranging post discharge services. The Social Worker functions as a member of the Care Connections multidisciplinary team with a goal of improving the patient’s health status and reducing the need for hospital care. Duties may also include assistance with program development, evaluation, professional education, or supervision of students and BSW staff.
Master’s degree in social work from a School of Social Work accredited by the Council on Social Work Education (CSWE)Knowledge of theory and practice of social work at the MSW level and have basic research skillsMust have the ability to work as part of a multidisciplinary team and with patients of various ages and cultural backgroundsMust have excellent written/verbal communication skillsEXPERIENCE: Prefer prior experience in a healthcare setting (i.e., hospital, home health or nursing home) but not required.
SPECIAL SKILLS: Excellent communication, interpersonal, organizational and presentation skills.
OTHER: Computer skills, Basic typing skills
