PHC’s Homeless Support Services is the entry point for serving people experiencing homelessness in Polk County. Enabling services are available to help patients with benefits enrollment, case management, transportation, translation, and patient education. We currently have locations in Ames, Des Moines, & Marshalltown. As a Patient Access Specialist, you will be responsible for providing excellent customer service for the various grant-funded programs within the HIV department. Responsibilities are performed working with electronic health records and scheduling systems delivering over the phone assistance to patients confirming and scheduling appointments and identifying needs. Coordinates with PHC and program team members to address identified needs. Follows assigned workflows to ensure maximum patient access and organizational success. What You Will Do + Reviews patient schedules to identify patients with upcoming appointments who need assessment for financial assistance, payor sources or other available program benefits. Performs insurance verification prior to patient appointments to confirm active coverage for the visit. + Completes robust confirmation calls to scheduled patients to identify and correct inaccuracies in appointment scheduling, demographics and payor source prior to scheduled appointments. Reminds patients to bring necessary items and documentation to appointments as needed. + Identifies care coordination needs through conversation with patients and notifies the care team of anticipated needs to help overcome barriers in making or keeping appointments. Arranges appointment transportation. Proactively arranges wellness cards in collaboration with the care team. + Responds to needs for patients being seen in, or walking into, the clinic with ability to triage needs and coordinate care with assigned care teams, including scheduling appointments with case managers. + Appropriately reschedules appointments as need is identified to ensure optimization of the clinic schedule. Contacts patients due for recalls to assist in retention in care and quality care provision. + Acts as the primary point of contact for patients utilizing Prevention Services seeking clinical care for related services. Collaborates with the Program Support Worker as the point of contact for patients requesting Project services. + Utilizes electronic medical records (EMR) and CareWare to document patient interactions timely and completely. Enters program data and services into EMR and CareWare, maintains and runs reports, and assists in the Ryan White Part B Quarterly Report as needed. + Assists program staff with consumer involvement activities, patient education, client events and on-going client satisfaction assessment activities. + Performs other duties as assigned. Qualifications You Need to Bring Required: + High school diploma or equivalent. + Two years experience in healthcare, human services, or finance; or three years in customer service. + Excellent customer service skills and commitment to service excellence. + Effective verbal and written communication skills. + Strong problem-solving skills with ability to apply critical thinking. + Ability to take initiative and demonstrate dependability. + Team oriented with ability to collaborate effectively with others. + Listening and reading skills with ability to follow written and verbal instructions. + Math & reading skills with the ability to follow instructions in all forms and calculations. + Basic computer skills with proficiency using Microsoft Office applications, keyboard entry and internet-based applications. + Ability to learn and effectively use the electronic health record system. + Detail oriented with a high degree of accuracy. + Organization, prioritization, and time-management skills. + Strong interpersonal skills with ability to establish and maintain effective working relationships with a diverse group of individuals. + Demonstrates professionalism with ability to remain calm in all situations and show empathy. Preferred: + Experience in HIV/AIDS care or with other vulnerable populations, grant driven organizations, community health centers or public health. + Bilingual Spanish & English verbal and written communication skills. + Experience with patient registration systems and electronic health records. + Experience using a multi-line telephone system. + Experience working with and knowledge of clinical terminology and health insurance We Take Care of Our People Your experience and skills determine your base pay. The hiring range for this position is typically $15.50 - $19.38 per hour/annually. Candidates with extensive work experience related to this position may be considered for additional compensation up to the pay grade maximum of $23.25 per hour/annually.
