Pay range:  $17/hr - $22/hr  based on experience

Hours of Operation:  2 week days:  8:30am - 5:00pm

 

JOB SUMMARY

 

The Patient Care Advocate is responsible for educating patient/family/caregivers on what to expect and explain common signs and symptoms of death.Performs duties that include active listening, education, documentation, and collaboration/communication with the Interdisciplinary Team to assist patients and families with their goals and assist the team in the delivery of hospice care to the patient, family, and caregivers.Functions as a liaison between patients, families, caregivers, and team member.Acts as an advocate in guiding patients/families/caregivers to necessary and desired services.

 

Qualifications

 

Education/Regulatory Requirements:High school diploma or equivalent required.Death Doula certification or similar certification or end of life team experience.Skills:Skilled in written and oral communications.Works effectively and with flexibility as part of a team.
