Patient Care Coordinator

Position Summary

The Patient Care Coordinator works with the Director and providers to ensure the processes for the clinic run efficiently. Assists physician with patient care, takes vital signs, completes registration process, collects patient identification and demographics, answers phones, schedules appointments, maintains a fax queue, and receives co-pays and ROA’s. This position is responsible for managing the information system and provides clerical and receptionist support, including but not limited to: transcribes physician's orders, answering phone, scheduling appointments, Compiles and maintains various records.

Responsibilities

Develop, organize, execute, and maintain the patient, nurse and provider flow for daily office. Provide direction of scheduling emergent patients with providers. Maintain a positive work environment with all staff to ensure all staff complete job duties, efficiently and in a respectful manner. Collaborates with materials management to ensure appropriate office supplies are available and stocked in the clinic. Maintain proficiency with the EMR software. Collects patient identification and demographics, HIPAA forms, insurance information, collects co-pay. Documents indicated signs/symptoms and consent for treatment. Completes and maintains registration process. Schedules appointments, tests, and procedures. Sends patient communication regarding appointments/tests/procedures. Answers phone, screens and directs caller’s, retrieves records from previous EMR. Prepares and maintains files/scans records. Maintains cash drawer, including copays and paid-on-accounts. Assists patient with accounts and billing problems or questions. Verify & maintain insurance authorization. Assists and completes prior authorizations for insurance. Cleans and returns all equipment to the proper storage area and removes any non-functioning equipment and reports appropriately. Completes and maintains all MSP, Kancare eligibility, VA registration. Documents vital signs and patient response to intervention/therapy and test results. Maintains patient exam rooms for evaluation of patients for nurse and provider. Maintains reception/waiting area. Sends patient communication regarding appointments/tests/procedures. Opens and prepares the waiting room to receive patients each day. Closes waiting room each day, which also includes the walking through waiting room to ensure no patients or items are left behind.

Required:

High School Diploma BLS certification Additional certification may be required for certain patient care settings or populations. Specific information and the time frame in which such certification must be attained will be provided by the department director/manager/supervisor at the time of hire/transfer into the department.

Preferred:

Successful completion of a nurse assistant training program. Knowledge of general nursing assistant functions. Current nursing assistant certification in the State of Kansas. Three years of clerical experience Basic computer knowledge of basic information system operations

Patient Interaction: Frequent

Infection Control: Initial and Ongoing training in dealing with infection control. Trainings could include but are not limited to, blood-borne pathogens, bodily fluids and biohazardous materials as it applies to your daily work environment.

HIPAA: This position will have access to the following Protected Health Information in order to carry out the duties related to their position at Hays Medical Center based on the following criteria:


Primary – required (routine) to do the job;
Secondary – required for the job, but mostly be exception; and
None – no approved access

Clinical Information (information that describes a patient’s health status): Diagnosis, Reports/Medical Notes, Test Results, Problem List, Procedures, History and Physical

Secondary: Coding Information (clinical information that is in (alpha) numeric format): ICD-10 Codes, Rev Codes, CPT Codes

Financial Information/Insurance (information related to insurance, billing and payment): Billing Information, Payer Name, Payer ID, Account Balances, Plan Elements Covered, Payment Information, Payment Rates
