Fairview Range is hiring a Patient Care Facilitator for our Emergency Room team in Hibbing, MN. The Patient Care Facilitator will be Coordinating activities related to Trauma Program designation with the Minnesota Department of Health. Conducting trauma chart review and process improvement activities in collaboration with the Trauma Program Medical Director. Identifying trauma-related educational needs for the Emergency Department staff and communicates to Manager of Emergency Services. Attending regional and state Trauma Advisory Committees as required. Disseminating evidence-based practices surrounding trauma care to both staff and providers. Serving on the hospital Trauma Committee. Schedule: This position will work 72 hours per two weeks. As a Patient Care Facilitator, you will be responsible for: Assessing trauma care rendered through chart review, in accordance to established trauma program standards and policies. Demonstrating awareness of age-specific criteria in obtaining assessment data. Identifying patient needs through case review and uses data to further develop trauma program standards and policies. Analyzing trauma case reviews as the basis for process improvement planning. Identifying expected outcomes which are realistic, measurable and attainable. Attending and actively participates in regional and state trauma advisory committees, using information obtained for use in the hospital's trauma program planning/development. Ensuring trauma program policies and standards reflect age specific care in collaboration with collaborating disciplines. Evaluating trauma cases and compares to the expected elements of performance outline by the Minnesota State trauma program. Acting as a role model utilizing personal strengths and weaknesses to direct the plan of care consistent with patient needs. Participating in ongoing informational and/or educational activities related to clinical knowledge and professional issues. Facilitating the trauma program through collaboration with the trauma committee on developing policies, procedures, philosophy and mission. Interpreting, implements, develops, recommends for revisions and assists with the updating of policies/procedures. Collaborates with the trauma program medical director and clinicians. Representing the Trauma Program both internally and externally. Participating in the budget process, including projection of educational programs. Monitoring adherence to standards and policies; reports improvement opportunities to ED Manager. Required Qualifications Minimum of one year of Trauma Program Manager and Stroke Coordinator experience. Critical Care and/or Emergency Nursing experience within the last 5 years. Minimum of one year experience in Medical/Surgical nursing required. Current Minnesota RN licensure. Current BLS certification required and maintains certification. Current ACLS, TNCC, PALS certification or attains certification within three months of employment and maintains certification. Preferred Qualifications Certified Emergency Nurse (CEN) BSN Course in Advanced Trauma Nursing (CATN), Comprehensive Advanced Life Support (CALS), Advanced Trauma Course for Nurses (ATCN), Emergency Nursing Pediatric Course (ENPC).
