Summary:

The Patient Care Navigator provides administrative and clinical support to nurses, social workers, manager of Population Health/Quality and Director of Population Health/Quality. This role will utilize care management processes established by the Population Health program but is not limited to Population Health.


Experience/Education Requirements:

HS Diploma or GED required, Associates degree preferred.

 Knowledge, Skills, and Abilities Required:

HS Diploma or GED required, Associates degree preferred Previous experience in an ambulatory care setting Strong written and verbal communication skills required Exceptional interpersonal skills required Strong ability to multi-task EMR experience (Epic experience preferred) Bilingual highly desirable Intermediate proficiency with Microsoft Office programs Ability to function independently, to organize, and be self-motivated

Responsibilities:
Daily monitoring of payer portals and reports to find inpatient discharges, Emergency Room discharges, or other high-risk patients. Provide patient outreach and assistance as it pertains to patient care flows according to policy and procedures. Document all patient communication and related activities in the Epic system and payer portals when appropriate. Monitor and review revenue, compliance, and communicate regarding scheduled appointments for transitional care visits with appropriate Site Supervisors. Work with patients and interdisciplinary care team to identify, educate, and alleviate extrinsic barriers to adherence (i.e. transportation, food insecurity, social services, financial concerns, pharmacy). Liaison to multiple departments, agencies, and provider offices (i.e. Loving Lift, inpatient social services, Mobile Integrated Health, Home Health Care, Primary Care offices, Internal Medicine offices, Pulmonology, Cardiology, Heart Failure Clinic, Coding/Billing). Monitor, evaluate, and document monthly care management readmissions and patients utilizing Home Health Care, Mobile Integrated Health and Loving Lift transportation. Identification, outreach and follow up in patients with gaps in preventative health maintenance and performs quality measure care gaps outreaches. Outreach to patients to ensure follow up visit with their PCP post inpatient or ER visit to the hospital. Provide education to patients on how to effectively manage their diagnosis at home (e.g. COPD stoplight education). Primary point of contact for designated for patients with chronic special needs and other identified “high risk” customers needing care management services. Mails, collects, scans and telephonically assists customers to complete health risk assessments. Assist in other administrative duties as needed.
