Patient Care Navigator
Overview Good Samaritan University Hospital is a 537- bed (including 100 nursing home beds) teaching hospital and a member of Catholic Health. Job Details Working collaboratively with the Case Managers, Social Workers and Transitions of Care (TOC) Navigators, the Patient Navigator's primary focus is to educate and connect patients in and to primary care usage, overall wellness and accessing community services. Job Requirements: • Knowledge of principles and processes for providing patients with information and an understanding of needs assessment. • Be knowledgeable about community resources appropriate to needs of patients/families. • Strong communication and motivational skills. • Experienced in managing assigned caseload of patients. Education and Experience: • High School Diploma or its equivalent. Bachelor's degree in Health Care, Population Health or other related discipline preferred. • One to two years working in a health care or community based organization preferred. • Strong leadership qualities, problem solving and analytical skills. • Written and oral fluency in English; Spanish a plus. • Basic computer skills. • Good communication skills, such as listening well, and using language appropriately.
