Patient Care Navigator - Albany - Full-time

Description:Position Summary:

St. Peter’s Cancer Center is seeking a compassionate and skilled Patient Navigator to provide social work services, care coordination, and community resource support for cancer patients and their families. The Patient Navigator will assess patient needs, identify barriers to care, and ensure patients access appropriate resources.

Key Responsibilities:

Psychosocial Needs Assessment:
Serve as a point of contact for patients and caregivers, conducting assessments to understand their medical, emotional, and financial needs. Develop care plans, coordinate services, and guide patients to appropriate resources.

Resource Assessment and Referral:
Identify patients' needs for support services, assist in overcoming barriers to care, and advocate for patients in accessing community resources and insurance coverage.

Program Development and Education:
Contribute to educational programs and participate in interdisciplinary cancer care meetings to improve patient outcomes.

Documentation:
Maintain accurate and up-to-date documentation of care and services provided in the Electronic Medical Record (EMR).

Qualifications:

Education:

Required: Bachelor’s degree in Social Work, Human Services, or a related field.Preferred: Master’s degree in Social Work (MSW).

Experience:

1-2 years of healthcare experience is preferred.Strong knowledge of cancer-related community resources.Excellent communication and interpersonal skills.

Skills:

Ability to work independently and as part of a team.Experience in case management and providing emotional support to patients and families.Why Join Us?

Pay Range: $21.20-$28.26

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
