*Description*
Summary:
The Patient Concierge Associate is responsible for managing the Discharge Lounge while welcoming and greeting patients and meeting the patient's needs as they wait to be discharged from Hospital. This role will manage the patient experience while in the lounge and ensure a smooth transition from the hospital.
Responsibilities:
1. Meet and Greet patients in a professional and courteous way
2. Assure patients are comfortable and their needs are supported
3. Ensure the lounge area is maintained and presented to the highest possible standard
4. Responds quickly to patients requests and manages the patient experience
5. Log and maintain accurate records of each patient visit
6. Monitors and assists discharged patients and ensure discharge procedures are being followed
7. Round on patients in units assuring excellent customer service to increase loyalty
8. Provide feedback to Director on patient issues that need to be addressed.
10. Maintains and Models Nuvance Health Values.
11. Demonstrates, regular, reliable, and predictable attendance.
Education: HS GRAD/EQUIVALENT

* Flexible and Ability to adapt to change and work in a team based environment
* Effective oral/written/verbal communication and customer service skills.
* Proficient computers skills including Microsoft Word and Excel
* Proficient EMR skills
Working Conditions:
Manual: Some manual skills/motor coord & finger dexterity
Occupational: Little or no potential for occupational risk
Physical Effort: Medium to Heavy effort. May exert up to 35 lbs. force
Physical Environment: Generally pleasant working conditions
