Job Title: Patient Outreach and Transition Support Specialist
Job Description

We are looking for a Patient Outreach and Transition Support Specialist to join our team. In this role, you will be responsible for contacting patients who are overdue for appointments, engaging in friendly conversations, encouraging and setting appointments, and addressing any concerns. You will also reach out to patients whose physician has left the practice, understand their new plan of care, and guide them to physicians similar to their previous provider. Additionally, you will verify and update patient histories, medical records, and relevant information using EMR/CRM systems, and identify and remove records of deceased patients or those who have moved out of coverage areas. Collaboration with other clinics to assist with appointment scheduling and logistics is also a key part of this role.

This is a contract position with a duration of 9 Month(s).

Work Site

This is a fully on-site position in Kansas City, Missouri.

Work Environment

You will sit out of Research Medical Center, KCMO, in an office setting environment (not sitting in an active clinic). You will have daily, weekly, and monthly goals for metrics with inbound/outbound calls, setting appointments, and updating patient profiles. We will review data monthly to ensure it is making an impact with success. You will be on the phone with various patients, some of whom may be upset about their provider leaving or having to see a new doctor. You will need to be able to de-escalate situations with a calm and cool demeanor and use critical thinking to be understanding and quick with resources.
