Patient Logistics Coordinator
Description We are seeking a committed Patient Logistics Coordinator to join our team in the healthcare industry located in Phoenix, Arizona, 85013. In this role, you'll play a crucial role in managing customer applications, maintaining accurate records, and handling customer inquiries. You'll also monitor customer accounts and take necessary actions. This role offers a contract to permanent employment opportunity. Available shifts (12-hour shift, 3x a week - looking for availability 7 days a week) 1) 7am-7:30pm 2) nightshift 7pm-7:30am Responsibilities: + Work independently to facilitate the placement of patients as directed by the Patient Logistics Manager, Patient Logistics RN, and physician orders. + Streamline the transfer of patients to the appropriate facilities and units. + Proactively identify potential problems and escalate them to the appropriate authority to prevent any barriers to timely patient movement. + Communicate seamlessly with staff at both referring and receiving facilities to ensure patients are placed at the appropriate level of care without delays. + Document all activities meticulously to promote effective communication with the care team members including Transfer Center, Hospital ED's, Hospital Units, Leadership, and receiving facilities. + Utilize outstanding communication skills to express concepts clearly and accurately to colleagues, customers, physicians, and care team members. + Providing and receive constructive feedback to/from colleagues with an open mind and a view to continuously improve. + Clearly articulate and uphold the Mission and Vision statements of the department. + Exhibit a customer-oriented philosophy, consistently striving to exceed expectations. + Participate in and encourage other team members to think creatively to accomplish duties. + Perform duties independently with minimal supervision.
