Patient Navigator, Infusion Intake

Primary Function/General Purpose of the Position

The Patient Navigator guides infusion patients through the healthcare system and removes barriers by assisting with access issues, developing relationships with service providers and tracking interventions and outcomes

Essential Job Functions:

Ensures timely patient access and seamless coordination of services throughout the continuum of  care for infusion; facilitates rapid scheduling, change coordination, and  to ensure completion of recommended patient treatment plans; assists patients with identifying and eliminating potential barriers to care by referring patients to appropriate resourcesActs as a conduit to remove barriers to ensure patient continuity of care for infusion works collaboratively with stakeholders to ensure best patient outcomes; updates physicians on utilization issues impacting patient careCommunicates with all appropriate colleagues to ensure collaborative patient care;may perform staff education related to resource utilization

Employment Qualifications

Education Requirements:

Associates Degree - Nursing (required)

Bachelors - BSN (preferred)

Licensure: RN License Required

Minimum Years and Type of Experience: Minimum of two years of previous experience in an oncology or infusion services environment required

Other Knowledge, Skills and Abilities Required: Ability to effectively communicate with diverse audiences; strong organization and detail orientation skills; prior experience working in protocol/pathway driven environments; strong critical thinking skills

Other Knowledge, Skills and Abilities Preferred: Previous experience in care coordination or infusion authorization preferred
