Patient Navigator

Position Summary The Cancer Center Intake Patient Navigator is responsible for leading the Oncology and Hematology program that focuses on the intake of new patient referrals, coordination and prioritization of new referral appointments with Oncologist/Hematologists. The Intake Patient Navigator  is responsible for patient and staff education and communication throughout all key phases of new patient referrals and the navigation for our patients to be prepared to start chemotherapy and radiation treatments.  The Intake Patient Navigator will provide assistance to the Physicians with timely review of new patient referrals and obtain all information for the patient when they arrive to see our Physicians and begin their treatment plans.  The Intake Patient Navigator will assist the Physicians in managing testing and procedures that need to be completed timely for the patient to start chemotherapy or radiation treatments.

 

Minimum Qualifications Education Graduate of an accredited school of registered nursing.

3-5 years of Oncology Experience preferred

Credentials Must possess a current, valid RN license in state of practice, temporary RN license in state of practice, or compact RN license for current state of practice.  BLS certification is required.

 

Physical Demands Able to stand, walk, bend, squat, reach, and stretch frequently. Possess physical agility and adequate reaction time to respond quickly and appropriately to unexpected patient care needs. Needs adequate hearing and visual acuity, including adequate color vision. Requires fine motor skills, adequate eye-hand coordination, and ability to grasp and handle objects. Able to use proper body mechanics to assist patients in ambulating, transferring in and out of bed, chair or wheelchair. May be required to lift up to 25 pounds. Must use standard precautions due to threat of exposure to blood and bodily fluids. Needs ability to communicate effectively through reading, writing, and speaking in person or on telephone. May require periodic use of personal computer. Essential Functions Actively participates in monthly staff meetings and monthly Provider meetings. Ensures education, communication and quality throughout the referral process for oncology and hematology patients to the Caner Center. Ensures quality communication to patients, and patients families when referrals are sent to the Callahan Cancer Center.  Maintains data, volumes, diagnosis specific referrals and reports monthly to our providers in the Cancer Center.  This data should be reported to our Cancer Committee quarterly. Participates in community outreach and education efforts to promote and support the assigned specialty program and/or department. As necessary and required by the organization and other parties leads and organizes these outreach and education efforts. Other duties and responsibilities as assigned with heightened emphasis to those that pertain to enhancing the patient experience through direct patient, family and care giver contact.
