Family Health Centers at NYU Langone, a designated Level 3 Medical Home, is the largest federally qualified health center network in New York State, and the second largest in the nation. Position Summary:
We have an exciting opportunity to join our team as a Patient Navigator.

In this role, the successful candidate The Patient Navigator coordinates patient care services as well as provides outreach education and referral services to patients The Patient Navigator is responsible for carrying out tasks that are needed to execute the medical and support service plan Depending on grant requirements and population served the Patient Navigator may provide rapid HIV testing and pre and post test counseling

Job Responsibilities:

Oversees a caselaod of patientsassists in group activities and facilitation nbsp;Participates in trainings conferences meetings and quality improvement efforts as assignedEnsures patients make and keep appropriate health and social services appointments; accompanies patients to primary care appointments and to other health care and social service encounters as appropriateProvides education risk reduction counseling and skill building services to the patients as necessary per patient needs and grant requirementsPerforms entitlement assessments reassessments and follow upsCoordinates ongoing navigation and logistical support for appointment keeping reminders transportation and childcare arrangements as appropriateProvides critical information to other members of the health care team based on observations in the field or in patient encountersEducates coaches and empowers patientsDepending on needs may provide DOT Daily Observed Therapy ; if providing DOT must observe and record patient self administration of medicationsTimely data entryOther duties as assigned

Minimum Qualifications:
To qualify you must have a Bachelors degree or equivalent combination of education and patient navigation/case management experience e g Associates degree and two years experience high school diploma/GED and four years experience Past experience and employment in a hospital or health care setting preferred Fluency in Spanish strongly preferred

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $41,137.15 - $42,000.00 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
